Sponsored by CORSTRATA and hosted by the Post-Acute Care Symposium (PACS), the Post-Acute Care Wound Nurse Excellence Award is open to all nurses working in post-acute care settings. The second annual award will recognize one individual for his/her direct contribution to overall wound care excellence in a post-acute care setting.

Award Recipient:
Post-Acute Care Wound Nurse Excellence Award recipient will receive a complimentary registration to attend the Symposium on Advanced Wound Care Fall 2021, Post-Acute Care Symposium event in Las Vegas, NVC, October 30 - November 1, airfare (up to $500) and two nights hotel accommodations at the event hotel, Caesar’s Palace. one of the participating event hotels (selected by Corstrata).

Criteria
All nominees must currently be a licensed nurses in a post-acute care setting (Nurse Nominee must practice wound care nursing as part of his/her direct patient care within a post-acute care setting).
We are recognizing wound nursing achievements within the last 12-18 months within the post-acute care (home health, hospice, skilled nursing facility, inpatient rehab, etc.) setting. Entries will be judged on the following contribution(s) to the profession:
1. Nurse Nominee demonstrates excellence in direct patient wound care that contributes to positive patient outcomes. (40%)

Provides evidence-based wound prevention and care on a daily basis.

Serves as an advocate for the patient and family.

Provides patient and family with wound education.

Develops or contributes to the development of wound best practice.

Actively participates in wound outcomes quality improvement activities.
2. Nurse Nominee serves as a mentor to staff and nurses, providing guidance on wound care and skin integrity best practices. (30%)

Coordinates formal training/continuing education to enhance wound care knowledge.

Develops/supports an integrated, team approach to wound prevention and management.

Assists in the orientation of new employees on wound care.
3. Nurse Nominee seeks opportunities to evaluate and implement new wound care products and services that ensure best practices and improve care quality. (20%)

Develops or contributes to the development of the wound formulary to support evidence-based care.
4. Nurse Nominee participates in community-based education programs that promote and advance wound care and skin integrity best practices among the patient population. (10%)

Entries should not exceed 1,000 words. Supporting materials are recommended for individuals self-nominating, including 1-2 co-worker/patient testimonials. All entries must conform to HIPAA requirements; all patient testimonials/anecdotes are to be anonymous with no identifying patient information included.
Entry Fees
The Post-Acute Care Wound Nurse Excellence Award requires no entry fee. No purchase is necessary. A purchase of Corstrata solutions or a 2021 Post-Acute Care Symposium (PACS) registration pass does not enhance your chances of winning.
