Supporting Hospital WOC Nurses with Care for their Ostomy Patients

Hospital WOC nurses often find it challenging to continue supporting their ostomy patients after discharge, leaving them without essential guidance for managing their ostomies. To address this concern, Ostomy 101 has partnered with Corstrata to offer virtual clinical consultation services to ostomy patients in their homes. Through this collaboration, hospital WOC nurses can refer their patients to Corstrata's team of WOC nurses, who will provide personalized support and education.

 

Corstrata's specialized ostomy nurses assess each patient's needs, recommend supplies, and teach self-care skills. This partnership allows hospital nurses to extend their support beyond the hospital, ensuring patients receive consistent, high-quality, post-discharge care without increasing hospital nurses' workloads. The virtual solution complements the care provided by ostomy nurses, enabling a seamless transition and optimal outcomes for patients adjusting to life with an ostomy.
